Arcanobacterium haemolyticum has an established role in the etiology of human pharyngitis. There are increasing reports of systemic infections caused by this organism. From India, we report the first case of Arcanobacterium haemolyticum causing pyothorax in an immunocompetent adolescent male patient. The probable mode of infection is also discussed.
Throat culture is the gold standard for diagnosing streptococcus pharyngitis. G streptococci, Fusobacterium necrophorum, Arcanobacterium haemolyticum, Arcanobacterium haemolyticum has been implicated mainly in non-streptococcal pharyngitis and wound infections. Rarely, it has been reported to cause syste. 2. Banck G, Nyman M: Tonsillitis and rash associated withCorynebacterium haemolyticum. Its treatment consists of supportive care and antibiotics, resulting in a good prognosis without any long-term sequelae when recognized and treated promptly. Arcanobacterium haemolyticum was recovered from 0.5% of throat cultures of 3,922 patients seeking medical attention because of sore throat. Most of the patients positive for A. haemolyticum were 15-25 years old, and had fever (80%), lymphadenopathy (67%), pharyngeal exudate (69%) or skin rash (23%). Neck pain or swelling due to regional lymphadenopathy commonly accompany sore throat. Fever, headache, fatigue, and malaise are variably present. The specific microbiologic cause of pharyngitis can rarely be distinguished based on clinical features alone.

The first cases ascribed to this pathogen were described in 1946 and caused ulcerative skin lesions among American soldiers in the Pacific Islands. A. haemolyticum is reported as an in- frequent cause of peiitonsiDar abscess, pharyngitis, and tonsiDitis in children and young adults (>4). It was cultured from 12 of 129 children with pharyngotonsilli tis or scarlet fever, some of whom had a scarletiniforrn rash (5). No controlled trials of antimicrobial therapy have been conducted for pharyngitis caused by A haemolyticum. Many patients with pharyngitis have been treated with 1 dose of intramuscular penicillin Infection with Arcanobacterium haemolyticum, a gram-positive rod, causes pharyngitis, typically in adolescents and young adults. Acute pharyngitis attributable to Arcanobacterium haemolyticum often is indistinguishable from that caused by group A streptococci. Fever, pharyngeal exudate, lymphadenopathy, rash, and pruritus are common, but palatal petechiae and strawberry tongue are absent.
